Coronavirus (COVID-19) poses a serious threat to older adults and those with underlying health conditions. This is why the federal and state governments are restricting visitors to nursing home communities. Exceptions may be made for end-of-life visits on a case-by-case basis. We understand this is difficult, but the safety and wellbeing of your loved one is our top priority.
The Recreation Department has been working diligently to maintain lines of communication with your loved ones through phone and video chat. To schedule a video chat with your loved one please email the Director of Recreation, Joan Erdmann at jerdmann@BerkshireRehab.com or reach her at extension 3316.
We have mobilized our Family Call Program where social work and activities staff regularly call our families with updates about their loved ones. As always, if you have any questions concerns or inquiries for the Social Work Department, please contact the Director of Social Services, Stacey Ann Morgan, at smorgan@berkshirerehab.com or extension 3309, or Jennifer Paulicelli at jpaulicelli@berkshirerehab.com, or extension 3310.
We are doing everything possible to ensure we stop the spread. Our staff and residents continue to follow the following recommended preventative actions within the building until the virus has been eradicated:
We are committed to communicating with family members the status of the virus in the facility. If any resident tests positive for COVID-19, any resident suffers a COVID-19 related death or if three or more residents or staff with new-onset of respiratory symptoms occur within 72 hours, we are committed to update the table below (as per Executive Order 202.18, 42 CFR 483.80 and CDC guidance). Due to patient privacy laws, we are unable to share any further specific information about the confirmed cases or deaths in our facility, but rest assured that we have notified their immediate family members or guardian.

VACCINE UPDATE
We are happy to announce that the facility has teamed up with ProCare Pharmacy to administer the COVID-19 vaccine to protect our residents and staff from COVID-19. All COVID-19 vaccines currently approved by FDA and CDC are constantly available to be provided at the facility, as well as booster doses (5 months after a completed regimen of Pfizer/BioNTech or Moderna vaccine series, or 2 months after a Johnson & Johnson vaccine). If you have any vaccine related questions, please do not hesitate to reach out to Theresa Dluginski, RN at Tdluginski@berkshirerehab.com or at extension 3327.
Coronavirus (COVID-19) poses a serious threat to older adults and those with underlying health conditions. This is why, effective January 12th 2022 the New York State Department of Health requires all visitors to provide proof of a negative COVID-19 test upon entry to the facility. The negative result may be taken within no more than one day of visiting for rapid antigen tests, or no more than 2 days prior to the visit if the test was done via PCR. Berkshire does supply rapid antigen self-tests at the door to the facility, and we are proud to continue offering these tests so as not to convenience our visitors so long as supplies last.
Please be reminded, that even though all visitors have provided proof of a negative test, surgical masks must still be worn when indoors in the facility at all times.
During the summer months, our patio is available throughout the day during regular visiting hours (weather permitting).
We also offer video chats with your loved one if you cannot make it in to visit for whatever reason, whether via FaceTime, Zoom, or Skype. To schedule a video chat with your loved one please email the Director of Recreation, Joan Erdmann, at Jerdmann@berkshirerehab.com or reach her at extension 3316.
We have mobilized our Family Call Program where social work and activities staff regularly call our families with updates about their loved ones. As always, if you have any questions concerns or inquiries for the Social Work Department, please contact the Director of Social Services, Lauren Gervasio, at Lgervasio@berkshirerehab.com or extension 3309, or Samra Rashid at Srashid@berkshirerehab.com or extension 3310.
As always, should you ever have any questions, concerns, issues, or advice, our Director of Concierge Services, Smantha Latorre, is always available to address even the simplest of needs. Samantha can be reached at Slatorre@berkshirerehab.com or extension 3311.

Thank you, and please don’t hesitate to reach out to us with any questions or concerns. We will continue to keep family members up to date as this situation unfolds, so they have the latest information about their loved ones.
